RRRT moving towards integrated care

Richmond Response and Rehabilitation Team works in close partnership with acute discharge colleagues, GPs, social care, mental health services and non-statutory partners, leading to better care and happier patients.

When services work in isolation, patient get ‘lost’, essential care is delayed and patient satisfaction declines. Working together avoids people having to repeat their story to different professionals or waiting for one part of the jigsaw to move on in their care.

Communication is essential. When different parts of the system understand what each other does, they can work together better and provide more holistic care that keeps people well and prevents them from being bounced around from service to service.
